Coverage

Coverage is dictated by your medical benefit plan that is usually available in your human resources department. To inquire about coverage, please contact your human resource department or your Third-Party Administrator (TPA) at the number indicated on your membership card.

Health Insurance ID Cards
Your LUTHERANPreferred Membership ID Card is your “key” to the quality healthcare services arranged for you by your employer. It is as important as your driver’s license and should be kept with you at all times. Destroy old, outdated ID cards, and always present your new card with the LUTHERANPreferred logo whenever and wherever you need care.

Your ID card will also contain important information such as phone numbers necessary for checking coverage, pre-certifying procedures, and information on co-pays.

Pre-Certification
Refer to your ID card for appropriate information. If the information about pre-certification on your ID card is unclear, contact your human resource department or Third-Party Administrator (TPA) for instructions. The TPA’s phone number will also be found on your ID card.

Claims
LUTHERANPreferred physicians and hospitals will file claims for you. LUTHERANPreferred will reprice these claims to insure maximum value to you and your employer. Once we have received the claim and repriced it, the Third Party Administrator (TPA) or insurance company will process the claim and arrange for payment to the provider. Once the claim has been paid, the provider may then bill you for any services not covered.
